Breast Cancer

What is breast cancer?

Breast cancer is a prevalent form of cancer globally, affecting approximately one in eight women during their lifetime. It can manifest in one or both breasts and may occur in various parts of the breast tissue. Breast cancer is characterized by abnormal cell growth within the breast, which has the potential to spread to surrounding tissues. While the majority of cases occur in women, it’s important to note that men can also develop breast cancer, albeit less frequently.

What is Testicular cancer?

Testicular cancer typically affects men in their middle age, with an annual occurrence of four cases per one hundred thousand individuals. It originates in the tissues of one or both testicles, which are situated within the scrotum, a loose pouch of skin located beneath the penis.
These tumors develop in germ cells responsible for sperm production, hence referred to as testicular germ cell tumors.

Risk Factors and Prevention

Certain risk factors, such as smoking and dietary patterns, can increase the likelihood of developing testicular cancer, while others, like hereditary factors, cannot be altered.

  • Undescended testicle/s
  • Family history of the condition
  • HIV infection
  • Previous history of testicular cancer
  • Body size, as well as race and ethnicity, may also play a role in susceptibility.
Screening for Testicular Cancer

Testicular cancer can often be effectively treated regardless of the stage of the disease. Typically, individuals detect a lump on the testicle either through self-examination or during a routine physical examination conducted by a doctor.

As there is no standardized screening process for early detection of testicular cancer, vigilance for symptoms is crucial. However, early detection facilitates simpler treatment, quicker recovery, and reduces the need for extensive chemotherapy or surgery, minimizing associated side effects. Therefore, it’s advisable to remain attentive to any signs of testicular cancer and regularly undergo routine medical check-ups.

Signs and Symptoms
Signs and symptoms of testicular cancer may include:
  • A painless pea-sized lump or swelling on the sides of the testicles
  • Heaviness and swelling of the scrotum
  • Discomfort or pain in the scrotum or testicle
  • Pain in the groin, lower stomach, and back
  • Presence of fluid in the scrotum
  • Tender or enlarged breast tissue
Diagnosis

If a lump or symptoms suggestive of testicular cancer are detected, doctors typically conduct the following diagnostic tests:

  • Ultrasound scan of the scrotum: This helps determine the location and nature of the lump on the testicle.
  • Tumor marker test: This assesses elevated levels of specific markers such as HCG, AFP, and LDH in the bloodstream.
  • Biopsy: A procedure involving the removal of a tissue sample for microscopic examination.
  • Surgical removal of the testicle: In some cases, surgery may be performed to remove the affected testicle for further examination.
  • Additional imaging tests: X-rays and CT scans may be conducted to assess the extent of cancer spread if testicular cancer is confirmed.
    It’s important to note that not all lumps are cancerous, and they may be caused by various other factors. Therefore, consulting a doctor for proper evaluation and diagnosis is crucial.

Sub-Types:
  • Germ cell tumor (GCT) is the most prevalent type of testicular cancer.

  • Nonseminomatous Germ Cell Tumor: This type is associated with infertility in men due to issues such as low sperm motility, reduced sperm counts, and abnormal sperm morphology.
  • Stromal tumors include Leydig cell tumors and Sertoli cell tumors.
Stages:
  • Stage 1
  • Stage 2
  • Stage 3
  • Stage 4
Treatment for testicular cancer typically involves various procedures and modalities:
  • Surgery:
    – High inguinal orchiectomy: Surgical removal of the affected testicle.
    – Retroperitoneal lymph node dissection: Surgical removal of lymph nodes in the abdomen.
  • Radiation Treatment:
    – Radiation therapy may be used in certain cases to target and destroy cancer cells.
  • Chemotherapy:
    – Chemotherapy involves the use of medications to kill cancer cells or prevent their growth and spread.
  • Chemotherapy with Stem Cell Transplant:
    – In some cases, chemotherapy may be combined with a stem cell transplant to enhance treatment effectiveness.

These treatment modalities may be used individually or in combination, depending on the specific characteristics of the cancer and the patient’s overall health condition. Treatment decisions are made in consultation with a multidisciplinary team of healthcare professionals to ensure the most appropriate approach for each individual case.

Life after testicular cancer treatment

Life after testicular cancer treatment involves regular follow-up appointments for the next five to ten years.

During the initial three years post-treatment, appointments are typically scheduled every two to three months. After this period, appointments may be scheduled every six to twelve months.

During these follow-up visits, doctors assess any potential side effects of treatment and inquire about the patient’s overall well-being. They conduct physical examinations to ensure the patient is coping well and may order blood tests, x-rays, and CT scans as part of the follow-up check-ups.

Furthermore, doctors remain vigilant for signs of cancer recurrence, emphasizing the importance of ongoing monitoring and surveillance to maintain the patient’s health and well-being.
