Breast Cancer

What is breast cancer?

Breast cancer is a prevalent form of cancer globally, affecting approximately one in eight women during their lifetime. It can manifest in one or both breasts and may occur in various parts of the breast tissue. Breast cancer is characterized by abnormal cell growth within the breast, which has the potential to spread to surrounding tissues. While the majority of cases occur in women, it’s important to note that men can also develop breast cancer, albeit less frequently.

What is Kidney cancer?

Kidney cancer occurs when kidney cells undergo abnormal proliferation and become cancerous. This cancer typically originates in the lining of the kidney and gradually spreads throughout the organ. It develops as healthy kidney cells in one or both kidneys begin to proliferate uncontrollably, forming clusters of cancerous cells.

Risk Factors and Prevention

Kidney cancer primarily affects adults, with a higher prevalence between the ages of 50 and 70, and it is more common in men than women.

Risk factors for kidney cancer include:
  • Tobacco use or smoking
  • Obesity
  • High blood pressure
  • Chronic kidney disease
  • Long-term dialysis
  • Family history of kidney cancer
Preventive Measures
  • Quitting smoking
  • Avoiding exposure to certain toxins or chemicals
  • Maintaining a healthy weight through balanced diet and regular exercise
Screening for Kidney Cancer

Some kidney malignancies can be detected early through screening tests, which can increase the likelihood of successful treatment with early diagnosis.

Screening methods for kidney cancer may include:
  • Urine test: Analysis of urine samples for the presence of blood or abnormal cells, which may indicate kidney abnormalities.
  • CT scan: Imaging test using X-rays to create detailed pictures of the kidneys and surrounding structures, useful for detecting tumors or abnormalities.
  • MRI: Imaging technique that uses magnetic fields and radio waves to generate detailed images of the kidneys, helpful in detecting tumors or abnormalities.
  • Renal ultrasound: Imaging test using sound waves to create images of the kidneys, which can help identify tumors or other abnormalities.
Symptoms & Signs
Kidney cancer may present with various symptoms and signs, including:
  • Blood in the urine (hematuria)
  • Back pain or discomfort in the loin area
  • Presence of a lump or mass on the back or side
  • Swelling of the ankles and legs
  • Elevated blood pressure
  • Low red blood cell count or anemia
  • Fatigue
  • Reduced appetite
  • Unexplained weight loss
  • Recurring fever unrelated to flu or other infections
Diagnosis

Kidney cancer diagnostic tests are used to determine the presence of the disease and its aggressiveness.

Tools and tests for diagnosing kidney cancer may include:
  • Biopsy: Removal of a small tissue sample for laboratory analysis to confirm the presence of cancer cells.
  • CT scan / PET CT: Imaging tests using X-rays or radioactive tracers to create detailed images of the kidneys and surrounding tissues, helpful in detecting tumors and assessing their extent.
  • Lab Tests: Blood and urine tests to evaluate kidney function and detect abnormalities that may indicate kidney cancer.
  • X-ray: Imaging test using radiation to create images of the kidneys and surrounding structures.
  • MRI: Imaging technique using magnetic fields and radio waves to generate detailed images of the kidneys and surrounding tissues, useful in evaluating tumor size and extent.
  • Cystoscopy and nephro-ureteroscopy: Procedures involving the insertion of a thin tube with a camera into the urinary tract to visually examine the kidneys, ureters, and bladder for abnormalities or signs of cancer.
Sub-Types:
  • Renal cell carcinoma
  • Urothelial carcinoma
  • Sarcoma
  • Wilms tumor
  • Lymphoma

There are over 30 different types of kidney cancer cells identified by pathologists.

Stages:
  • Stage 1: The tumor is 7 cm or smaller and has not spread to nearby lymph nodes or distant organs.
  • Stage 2: The tumor is larger than 7 cm but has not spread to nearby lymph nodes or distant organs.
  • Stage 3: The tumor has reached nearby lymph nodes but has not spread to other body areas.
  • Stage 4: The tumor has extended beyond Gerota’s fascia and into other parts of the body.
Treatment Modalities
  • Surgical Oncology: Radical nephrectomy: Removal of the tumor, entire kidney, and surrounding tissue.
     – Partial nephrectomy: Removal of the tumor while preserving kidney function.
     – Laparoscopic and robotic surgery: Minimally invasive procedures using keyhole incisions and telescoping tools for nephrectomy.
     – Cytoreductive nephrectomy: Surgical removal of the kidney along with metastatic disease beyond the kidney.
     – Metastasectomy: Removal of tumors from other organs affected by metastasis, such as the liver, pancreas, or lung.
  • Medical Oncology: Radiofrequency ablation: Use of electric current to destroy cancer cells.
     – Cryoablation: Freezing of cancer cells using a metal probe inserted into the tumor.
     – Targeted therapy: Drugs like Anti-angiogenesis therapy, Bevacizumab, Tyrosine kinase inhibitors (TKIs), mTOR inhibitors, and HIF2a inhibitors.
     – Immunotherapy: Boosting the immune system to target and destroy cancer cells.
     – Chemotherapy: Use of drugs to kill cancer cells by inhibiting their proliferation and division.
  • Radiation Oncology: Radiation therapy is rarely used as a first-line treatment for kidney cancer and is not typically used alone.
Coping with Treatment:

In addition to managing the medical side effects of kidney cancer therapy, patients often face financial burdens and emotional and social challenges. Talking to a counselor or family member can provide valuable support and help patients cope with treatment.
