Breast Cancer

What is breast cancer?

Breast cancer is a prevalent form of cancer globally, affecting approximately one in eight women during their lifetime. It can manifest in one or both breasts and may occur in various parts of the breast tissue. Breast cancer is characterized by abnormal cell growth within the breast, which has the potential to spread to surrounding tissues. While the majority of cases occur in women, it’s important to note that men can also develop breast cancer, albeit less frequently.

Ovarian Cancer

What is Ovarian cancer?

Previously, it was believed that ovarian cancers originated exclusively within the ovaries. However, recent studies suggest that certain ovarian cancers may actually arise from cells located at the distal (far) end of the fallopian tubes. As a result, the term “ovarian cancer” is often used to encompass malignancies arising in ovarian, fallopian tube, or peritoneal cells. Despite their distinct origins, these malignancies are managed similarly due to their close association.

Risk Factors and Prevention

Women aged 50 and above face an increased risk of ovarian cancer.

Risk factors for Ovarian cancer include:
  • Being overweight
  • Delayed childbearing or never having a full-term pregnancy
  • Hormonal therapy after menopause
  • Family history of ovarian, breast, or colorectal cancer
  • Smoking
Preventive Measures
  • Breastfeeding
  • Childbearing
  • Use of birth control pills
  • Oophorectomy (surgical removal of the ovaries)
Screening for Ovarian Cancer
Screening examinations for ovarian cancer play a vital role in early detection, significantly enhancing the chances of successful treatment. Research indicates that 94% of patients diagnosed with ovarian cancer in its early stages survive for more than 5 years.
Screening tests include:
  • Transvaginal ultrasound (TVUS)
  • CA-125 blood test
Symptoms and Signs
  • Feeling of bloating
  • Abdominal or pelvic pain
  • Difficulty eating or feeling full quickly
  • Increased urgency or frequency of urination
  • Persistent fatigue
  • Backache
  • Constipation
  • Abdominal swelling accompanied by weight loss
Diagnosis

Diagnostic tests for ovarian cancer aim to determine the presence of the disease and its aggressiveness.

Tools and Tests:
  • Abdominal-pelvic examination
  • CA-125 assay
  • CEA
  • Biopsy
  • CT scan of the chest, abdomen, and pelvis
  • PET-CT scan
  • Transvaginal ultrasound
Sub-Types:
  • Epithelial ovarian tumor
  • Borderline epithelial tumors
  • Malignant epithelial ovarian tumors:
     – Serous carcinomas
     – Clear cell carcinoma
     – Mucinous carcinoma
     – Endometrioid carcinoma
  • Germ cell malignancies:
     – Dysgerminomas
     – Teratoma
     – Endodermal sinus tumors
     – Embryonal carcinomas
     – Granulosa cell tumors
  • Fallopian tube cancer
  • Primary peritoneal cancer
Stages:
  • Stage 1: Malignancy confined to the ovaries.
  • Stage 2: Spread to the fallopian tubes or uterus.
  • Stage 3: Involvement of lymph nodes along major abdominal blood vessels or peritoneum outside the pelvis.
  • Stage 4: Spread to organs outside the abdomen, such as the liver, spleen, or distant lymph nodes.
Treatment Approach
  • Surgical Oncology: The primary treatment for most ovarian cancers involves surgery. The extent of surgery required depends on the patient’s overall health and the extent of cancer spread. Surgical procedures may include:
     – Salpingo-oophorectomy: Removal of fallopian tubes and ovaries
     – Hysterectomy: Removal of the uterus and surrounding tissues. This may be partial or total.
     – Dissection of lymph nodes and lymphadenectomy: Removal of lymph nodes in the paraortic and pelvic regions.
     – Omentectomy: Removal of the thin tissue covering the stomach and intestines.
     – Cytoreduction or Debulking: Procedure for advanced ovarian/fallopian tube cancer patients.
  • Medical Oncology:
     – Chemotherapy: Utilizes chemicals to kill cancer cells, often using multiple drugs.
     – Targeted Therapy: Frequently used alongside chemotherapy for advanced ovarian cancer.
     – Immunotherapy: Boosts the immune system’s ability to combat cancer, explored in advanced cases where other treatments have failed.
     – Hormone Therapy: Used for advanced ovarian cancer that has spread beyond the pelvic cavity, involving medication to reduce hormone levels.
  • Radiation Oncology:
      – Radiation Therapy: Utilizes high-energy beams like protons and X-rays to kill cancer cells. While less common for ovarian cancer, it may be considered in specific cases.
Managing Treatment Challenges for Ovarian Cancer Patients:

In addition to grappling with the physical side effects of ovarian cancer therapy, patients also face the financial strain of cancer care and endure emotional and social impacts.
