Breast Cancer

What is breast cancer?

Breast cancer is a prevalent form of cancer globally, affecting approximately one in eight women during their lifetime. It can manifest in one or both breasts and may occur in various parts of the breast tissue. Breast cancer is characterized by abnormal cell growth within the breast, which has the potential to spread to surrounding tissues. While the majority of cases occur in women, it’s important to note that men can also develop breast cancer, albeit less frequently.

Cervical Cancer

What is cervical cancer?

Cervical cancer often manifests with the first sign being the presence of a lump triggered by either a human papillomavirus (HPV) infection or changes in healthy cells on the surface of the cervix. Prolonged HPV infection can lead to the development of a growth or tumor on the cervix.

Risk Factors and Prevention

While some individuals with no known risk factors develop cancer, others with several risk factors may never do so.

Factors that may increase the likelihood of developing cervical cancer include:
  • Human papillomavirus (HPV) infection
  • Immune system deficiency
  • Genital herpes
  • Tobacco use/smoking
  • Use of oral contraceptives
  • Multiple sexual partners
  • Exposure to diethylstilbestrol (DES)
Preventive Measures
  • Quitting smoking
  • Delaying first sexual intercourse until the late teens or later
  • Limiting the number of sexual partners
  • Using dental dams and condoms for safer sex practices
  • Avoiding sexual activity with individuals who have been in multiple relationships or exhibit signs of genital warts.
Screening for Cervical Cancer
Screening examinations for cervical cancer play a crucial role in detecting cancer at an early stage, increasing the likelihood of successful treatment.
  • HPV test
  • Pap test (Pap smear)
  • Visual inspection with acetic acid (VIA)
Symptoms & Signs
Symptoms of cervical cancer may include:
  • Very mild bleeding or blood spots before or after periods
  • Longer and heavier menstrual bleeding than usual
  • Bleeding after sexual intercourse or a pelvic examination
  • Abnormal vaginal discharge
  • Painful sexual intercourse
  • Bleeding after menopause
  • Persistent, unexplained back and/or pelvic discomfort
  • Very mild bleeding or blood spots before or after periods
  • Longer and heavier menstrual bleeding than usual
  • Bleeding after sexual intercourse or a pelvic examination
  • Abnormal vaginal discharge

Diagnosis
Diagnostic tests for cervical cancer aim to determine the presence of the disease and its aggressiveness. Various tools and tests used in diagnosis include:
  • Bimanual pelvic examination and sterile speculum examination
  • Pap test (Pap smear)
  • HPV typing test
  • Colposcopy
  • Biopsy
  • Pelvic examination under anesthesia
  • X-ray
  • CT or CAT scan
  • MRI
  • PET-CT scan
  • Biomarker testing

Sub-Types:
  • Squamous cell carcinoma
  • Adenocarcinoma
Stages:
  • Stage 1: Cancer is confined to the uterus but has spread from the lining of the cervix into deeper tissue.
  • Stage 2: Cancer remains within the pelvic region but has extended to adjacent tissues such as the vagina or nearby cervix tissue.
  • Stage 3: The tumor affects the lower part of the vagina and/or has spread to the pelvic wall, leading to hydronephrosis (swelling of the kidney), rendering one kidney inoperable, and/or involving local lymph nodes.
  • Stage 4: Cancer has metastasized to organs such as the rectum or bladder, among others.
Treatment:
  • Surgical Oncology:  – Conization: Removal of all abnormal micro-invasive cancer tissue.  – Loop electrosurgical excision procedure (LEEP): Removal of micro-invasive cervical cancer tissue.  – Hysterectomy: Removal of the uterus and cervix.  – Radical trachelectomy: Removal of the cervix, preserving the uterus.  – Exenteration: Removal of the uterus, vagina, lower intestine, rectum, or bladder if cervical cancer has spread to these organs.
  • Medical Oncology: – Chemotherapy:  Concurrent chemoradiation: Preferred treatment for various stages of cervical cancer, combining radiation and chemotherapy.  – Advanced or recurrent cervical cancer: Chemotherapy may be used for cervical cancer that has spread to other organs or tissues, or for cancer that returns after chemoradiation treatment.
  • Radiation Oncology:  – Radiation therapy: Used as part of primary treatment or for cervical cancer that has spread or recurred, depending on the stage of the disease.
Coping with Treatment:

In addition to managing the medical side effects of cervical cancer therapy, patients often face financial burdens and emotional and social impacts. Seeking support from a counselor or family member can be invaluable in coping with treatment.   • Avoid consuming processed meats, which may have adverse effects on your health during treatment.
Post-Treatment Support

Cervical cancer survivors may experience long-term side effects of surgery, radiation therapy, chemotherapy, and hormonal therapy. Common symptoms include menopausal symptoms such as hot flashes, joint pain, fatigue, mood changes, depression, and anxiety. Survivors benefit from empathy, mental strength, and support from their families. Additionally, joining post-treatment survivorship support groups can provide valuable understanding and assistance during the recovery journey.

Follow-up Cancer Care Plan:

Following treatment, it’s crucial to request a follow-up care plan from your healthcare team. Doctors will develop a personalized plan tailored to the type and stage of cancer you have undergone treatment for. This plan typically includes regular check-ups, screenings, and other medical interventions aimed at monitoring your health and addressing any potential recurrence or side effects of treatment.

Surveillance and Monitoring for Signs & Symptoms of Recurrence

One of the primary objectives of follow-up care is to monitor for any signs of cancer recurrence. It’s essential never to miss scheduled follow-up visits and to promptly report any symptoms that may indicate a recurrence. Cancer can reoccur because some cancer cells may remain undetected in the body, particularly in small locations that may not have responded to treatment. During follow-up care, a physician familiar with your medical history can offer personalized information regarding your risk of recurrence and appropriate monitoring strategies.
